Appointment Policy
Scheduling: Appointments can be made by calling 916-691-5233 or through our online contact form. We recommend scheduling in advance to ensure your preferred time slot.
Confirmation: We will call to confirm your appointment 24-48 hours in advance. Please ensure we have your current phone number.
Arrival Time: Please arrive 15 minutes early to complete any necessary paperwork and check-in procedures.
Cancellation & No-Show Policy
24-Hour Notice Required: We require at least 24 hours' notice for appointment cancellations or rescheduling.
Same-Day Cancellations: Same-day cancellations or no-shows may be subject to a cancellation fee to compensate for the reserved time slot.
Late Arrivals: If you are running more than 15 minutes late, please call us immediately. We may need to reschedule your appointment to avoid delays for other patients.
Emergency Situations: We understand that emergencies happen. Please call us as soon as possible if you have an emergency that prevents you from keeping your appointment.
Payment Policy
Payment Due: Payment is due at the time of service unless prior arrangements have been made.
Accepted Forms: We accept cash, credit/debit cards, and Health Savings Account (HSA) cards.
Insurance: We accept most major vision and medical insurance plans. Please bring your insurance card to every appointment.
Verification: It is the patient's responsibility to verify insurance benefits prior to the appointment. We recommend calling your insurance company to confirm coverage.
Privacy & HIPAA Policy
Confidentiality: We are committed to protecting your health information in accordance with HIPAA regulations.
Information Sharing: Your medical information will only be shared with authorized individuals for treatment, payment, or healthcare operations.
Access to Records: You have the right to access and request copies of your medical records. Please allow 24-48 hours for processing.
Communication: Please let us know your preferred method of communication (phone, email, text) and any restrictions on leaving messages.
Minor Patient Policy
Parental Consent: All patients under 18 must be accompanied by a parent or legal guardian, or have written consent from a parent/guardian.
Decision Making: The parent or legal guardian is responsible for making healthcare decisions and signing consent forms.
Payment Responsibility: The parent or legal guardian is responsible for all charges related to the minor's care.
Forms & Documentation Policy
DMV and School Forms: We can complete most forms requiring optometric assessment. Please inform us when scheduling and bring the form to your appointment.
Additional Testing: Some forms may require additional testing beyond a standard eye exam, which may incur additional fees.
Processing Time: Most forms can be completed during your visit. Complex forms may require 24-48 hours for completion.
Prescription Copies: Written prescriptions are provided at no charge. Additional copies may be available upon request.
